This form must be completed by a parent or legal guardian when a learner under 18 enrols in an AMBA SÔ LEARNING course.

Important Requirement

A learner under 18 cannot participate in classes or class recordings until valid parental consent has been received and matched with the learner’s registered account. Payment or course enrolment does not replace this requirement.

The learner’s full legal name and date of birth
The email address used for the learner’s AMBA SÔ LEARNING account
The learner’s batch, course and learning format
The parent or legal guardian’s contact information
The required enrolment and recording permissions
